Register to Become an Organ, Eye and Tissue Donor
- Nearly 8,200 New Yorkers are waiting for an organ transplant. In the United States, 17 people die each day waiting for an organ transplant.
- Only 46% of New Yorkers aged 18 and over have enrolled in the NYS Donate Life Registry, compared to the national average
of 63%.
- Just one organ donor can save up to eight lives and improve the lives of up to 75 people.

Once on that Page, fill out the required information:
You will need your current NYS Driver's License
When filling out the information on the application, the "Document Number" can be found on the back of your license, 2 lines above your name you will see "IDUSA" After that you will see 8-10 numbers and letters, this is your document number. After submitting that info and other personal info, you will receive an email, just follow the instructions and within a few minutes you will become an organ donor.
